Creating a Form

Maximize your event success by replacing outdated paper sign-in forms with Aidium Forms. These forms automatically populate your contact list with submitted information and, when paired with our automation tools, can initiate nurturing campaigns.

Creating a Form

* Indicates a required field.

  • Once logged into your Aidium account, click on Marketing in the left navigation menu.
  • Marketing will open up on the Forms page.
  • Click on + Create Form in the top right of the page.
  • Click on the Edit Icon (top left) and name your Form

  • Type: Select the type of form that you would like to use.
    • Contact form with email and form - This form includes first name, last night, email, phone number, and message field.
    • Co-Branding Consent- This form includes first name, last night, email, phone number, work phone number, job title, profile photo.
  • Partners: 
    • Select to include Loan Officer
    • Select to include Partner

Contact Settings

  • Contact Status: Select a contact status to apply to Contacts submitted by the form. This is optional.
  • Contact Tags: Click on Add Tag to select tags you would like to apply to submitted Contacts. This is optional.
    • You also will have the ability to create a new tag if one does not already exist. Click on + Create New Tag, enter the tag name, and click Create. The new tag will automatically be added to the form.
      • To remove a tag simply click on the x on the right hand side of the tag name.

  • Click on Create Form.
  • A pop-up will appear with the embed code. Aidium Lead Capture Forms are able to be embedded in any webpage. They are also available to be used in Aidium Sites. Please see Creating a Site for more information.

    • Clicking Copy code will copy the embed code to your computer’s clip board.
    • Clicking Back to Forms will close the Embed Code pop-up

 

Editing an Existing Form

  • Navigate to Marketing. Click on Forms on the top of the page.
  • Click the ... next to the form you’d like to edit.
  • Click on Edit.
  • Make any changes you need to make.
  • Click Save Changes.

Get Your Form's Embed Code

  • Navigate to Marketing. Click on Forms on the top of the page.
  • Click the ... next to the site you’d like to edit.
  • Click on Get Embed Code.
  • A pop-up with the embed code will appear.
  • Click Copy code.
  • Click Back to Forms once you have copied the code. You can now past this code in any website.

Deleting a Form

  • Navigate to Marketing. Click on Form on the top of the page.
  • Click the ... next to the site you’d like to edit.
  • Click on Delete.
  • A pop-up will appear and ask if you are sure you want to delete the form. Click on Delete to delete the site or Click Cancel to return to the sites page.

 

Adding a Form to Aidium Sites

  • When you are creating an Aidium Marketing Site you will be asked to select a form at the bottom of the page.
  • Any form you have created (contact or partner) will appear in this drop down. A new form can be used on an existing site. Follow the Instructions on the Creating a Site article for Editing an Existing Site.

 

Adding a Form to an Outside Site

Please note: Aidium Support is available to provide general information, but all actions should be carried out by your IT support. 


Using the embed code your newly created form can be embeded into any website. If your Enterprise manages your Loan Officer website then you will need to reach out to them and provide them with the embed code for your new form.

If you use a site like Weebly, Ionos, GoDaddy, Wix, etc you can add the form yourself.

  • Login into the service you use to manage your website.
  • Click to edit your site, selecting the page you would like to add the form.
  • Select and add an embed code box (the below example is from Weebly) and paste the embed code from Aidium.
  • Save your changes.
  • Click on Publish to push your changes to your live site.
